Before Your First Appointment: 

• Provide us with your name and email to connect you to the patient portal. 

• Complete new patient paperwork via the portal. 

• Complete questionnaire(s) (e.g. Sensory Profile-2) sent via email. 

 

 

Initial OT Evaluation: 

• A single appointment to determine your child's strengths and needs. 

• Attendance limit of one parent and one child if possible. 

• Assessments completed during the appointment. 

• Discuss the plan of care and treatment process if OT is recommended. 

• Approximately 60 minutes in length. 

• Schedule regular treatment appointments if desired at check-out.

Treatment: 

• Regular weekly appointments, typically lasting 45-60 minutes. 

• Please plan accordingly for commute and transition time into the clinic. 

• Co-payments due at time of service (check-out). 

• Please reach out if you need me to meet you outside or make other special arrangements. 

• You are welcome to stay or drop-off your child for their appointment. 

• Therapist will communicate with you if there are special circumstances (safety or other).
